Visit Us for a Consultation

person using a tabletYour initial consultation is a crucial step in the treatment planning process. Our office prides itself in detailed preoperative consultations so that we can deliver the utmost clinical care and anticipate surgical outcomes. As the consultation is conducted by our surgeons, this is a billable expense, typically covered by your insurance carrier with minimal out-of-pocket expense for the patient.

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, surgery can be performed the same day as the consultation. However, a complex medical history or a complex treatment plan will require a consultation/evaluation and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your consultation:

  • Any correspondence from your referring dentist/doctor detailing the nature of the services to be provided;
  • Any x-rays which may be utilized in the treatment process (our office has the capability to take additional x-rays when necessary);
  • If you have a medical and/or dental HMO policy, you will also need to provide an authorization from your insurance company;
  • A list of all medications you are presently taking (including prescribed, over-the-counter and herbal supplements);
  • Please present your medical and/or dental insurance cards at the time you check in for your initial consultation.

IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.

A pre-operative consultation and physical examination is mandatory for all patients undergoing surgery in our office. If you are undergoing IV sedation in our office setting, please note, you cannot have anything to eat or drink at least 6 hours prior to your surgery. You will also need an adult present in the office during surgery and able to transport you home following the procedure.

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)

Providing X-Rays

If your dentist or physician has taken x-rays, you may request that they forward them to our office. If the x-ray is relatively current and shows all details necessary for the procedure to be performed, our office can utilize your referring doctors radiographs. In the event you are scheduled within three days or less, we ask that you personally obtain your x-rays from your dentist/doctor and bring them with you to the initial consultation. If additional films are necessary, each office has the capability of performing diagnostic radiographs.
